MedStar Health Healthy Living program – Transforming health through everyday choices
MedStar Health is proud to offer a comprehensive program designed for those managing an ongoing chronic condition. This program empowers individuals to better understand their diagnosis while providing a warm, welcoming environment where they can connect with others facing similar health challenges. Through shared experiences and open conversation, participants gain both knowledge and support—helping them feel seen, heard, and cared for throughout their health journey.
The MedStar Health Healthy Living program emphasizes a lifestyle medicine approach—focused on preventing, managing, and even reversing certain diseases through positive behavioral changes. By replacing unhealthy habits with healthy ones, patients can improve their quality of life. Connecting via a shared telehealth medical visit offers a collaborative approach in an optimal setting for continued learning and sharing.
Program highlights
Patients who qualify for the program
Adults 18 and older who have been diagnosed with one of the following chronic conditions:
- Alzheimer’s/dementia
- Anxiety
- Depression
- High cholesterol
- High blood pressure/hypertension
- Memory loss
- Mild cognitive impairment
- Prediabetes
- Stroke
- Transient ischemic attack (otherwise knowns as mini stroke)
Program sessions
Sessions are eight weeks (four shared telehealth visits every other week) and each session is 60 minutes. To maximize program benefits, it is recommended that patients attend all sessions. The following sessions are part of the MedStar Health Healthy Living program:
- Brain Care
- High blood pressure/Hypertension
- Prediabetes
- Weight Management and GLP-1s
Team collaboration
Sessions are led by a clinical provider and a health and wellness coach to help motivate, inform, and educate patients and provide guidance around lifestyle medicine principles.
